I would suggest removing "Mingey" from your title as it is not needed and makes your application and you look slightly more immature. Just a little suggestion.

My opinions;
1) App looks good.
2) I don't remember your name. It means i can not see you in game.
3) Inactive, same with 2. Opinion.

You can be active when you got mod rank but you need try work harder for be mod. We can not know that. We need to see you active because of that.

Rule Knowledge: Bad( When i played with you acouple of months ago i thought you know the rules but after looking at your warns it seems that you either know the rules and you are braking them or you dont know the rules :/)
Application Effort: Good (Its looking good, i've read through it and the answers seem right)
Application Format: Good (You took the time to make the questions stand out and you added abit of colour to it)
Activity: Bad (I haven't seen you for a while ingame but your overall gametime seems good)

You can ask any superadmin for your training..
