If you're appealing your ban:

Rename your title to:

[Server you were banned from] 's Ban Appeal

And use this template:

You were banned for a day for the reason 12 recents, not for RDM. The ban is already over and you can join the server now. If you wish to appeal the warning that you're mentioning, then you need to make a seperate appeal for that punishment specifically.
