If you're appealing your ban:

Rename your title to:

[Server you were banned from] 's Ban Appeal

And use this template:

[b]Reason -[/b] What is/might be the reason that you got banned?

[b]Story -[/b] What happened?
[b]Proof -[/b] Any proof on a video/screenshot that clearly show what happened?
[b]Extra information -[/b] Any thing you think may be relevant to your ban.

You will only have a certain amount of time to change your topic to the template or it will be denied.

If you are reporting a Staff Member for abuse then rename your title to:
