After talking with Ross and looking at someone your past punishments and more recent punishments I've decided to deny this appeal. Your ban was 3 months ago but this is your 3rd permanent ban. I feel 3 months is way too short given your history. Beyond that you've been given many many many chances to do the right thing and come back and continue to get banned. I feel that you need to take this time to really decide if you wanna act properly. If you need to read the rules again you can read them here.

You can appeal again if you wish however, I think you should work on proving you can do the right thing first.
