Name - What is your ingame name?
STEAM-ID - What is your STEAM-ID?
Admins name - Who banned you/who might it be?

The steam ID is mine,you need to put your own steam ID in the template not the admin's steam ID that banned you.
I couldn't even understand a word what you said,improve your english maybe?
