You know it's crazy because I had a fairly similar idea today and I suppose the timing of me seeing this couldn't have been any better.
Anyhow, my version of this would be similar. Let's say, for instance, a user has been banned (10~) times for the reason (scamming), and let's suppose the staff team has decided that it is best if we keep the ban unchanged for an extended period of time. The staff member(s) could add a note to the ban saying "Do not unban before at least - (6 months) - have passed," essentially allowing a potential appeal to be denied without it being necessary to spend too much time talking to the staff member who originally elected to punish the user, if any time at all.
Obviously, in this specific situation, the note(s) are useful for clarifying the minimum time to wait before other staff members can start to consider unbanning the banned user in question. In which case you would probably want the note itself to stay on the rap sheet for at least (6~ months). However, notes could be set to last virtually as long as possible. There are also various different uses for these notes, such as "Verbally warned (2) times prior to the punishment that was given", etc. and any other information that might be generally helpful in understanding the actions of a user, or determining an appropriate punishment for the user.
As for the front-end of things, I believe punishments could have some form of an indicator that a note has been left, for instance like this (Latest punishment):
This is only a draft, though, and I'm open for better suggestions on how to tackle the front-end aspects of it.
This would not void the Reason/Type tab as it already serves its purpose to display the type- and/or reason for the ban (which would be something along the lines of "scamming," "hacking," etc.), whereas the notes would specify anything from the conditions of the ban to instructions for other staff members to read in the future. Notes would also be more detailed and generally longer, whilst the reasoning still stays short.
Anyway, I haven't gone through the details of this idea entirely, but I'm leaving it here for anyone to improve and elaborate on. I'm sorry if this turns out to be vastly similar to the original thread, but I'm drunk right now and I usually fail to do certain things.
