If someone breaks a rule, you need to punish them and then warn after. So in this case you'll gag/mute them and then give the warning after instead of waiting for him to do it again.

You should just wait for the next map instead of ungagging/unmuting them after a round. They'll automatically get ungag/unmute after map change.
You have loads of activity this week, but you should try and play on server 1, like Jim_Jam said. You should also change the things i've stated and maybe add some bold to the text. I've just met you today, so I can't really tell how you act in-game but i'm sure you'll do a good job.
