Well handling NLR situation is bad, it doesn't matter if player y got warned for nlr recently and you warn him again for that.
To know if the player broke nlr is simple, all you need to is go check the logs and see if player x killed player y twice or player x killed player y and player y comes back to kill him within 3 minutes. There is a timer in the logs though.
The situation for player x calls help in ooc, he actually broke meta, but you didn't give a reason why you verbal warn him.
The situation about rdm, witness doesn't count either, also Player x may lie in the report, because he may raided him or something like that.
Note:F1 is also for reports and F6 for showing players warning.
