Player x accuses player y for breaking NLR, the kills are pretty close together in console (a few lines apart) at a peak time.: On this one I would not warn player y straight away, I would ask for proof. If there was then I would warn player y. But if there was no proof then i would verbal warn player y.

You explained it very well but it can be in any part of the map and not only outside of your base (if you mean with outside of the base as the wholemap.)
"Metagaming"
You cant check if both guys are skyping, in my case i'd write "Joel asks for help on OOC to Jamie because..."
"NRL"
How do you know that player y really broke NLR and why'd you verbal warn him if you didn't know if was him or player x?
By my review on this application i'll end up with
Neutral you're active but i don't know if you've been recently warned, while admins post this thread with "recent warns or not" i might change my verdict to negative if recent warns or positive if no warns.
