TOPIC: Reason for denial

I've been trying to contact HAs about my recent punishment and about how it was supposed to be removed, yet I've had no response, so my app got denied for this. The person who gave it said he would contact someone to get it removed, but he failed to do so and wasn't present in the meeting. Can anyone explain this to me? Thanks.
