TOPIC: [CSGO] Ultimatum eSports Application Thread

Welcome to The Official Ultimatum eSports Application Thread!
This is a CS:GO Team created by fIZZAY on 23/12/2015 that hope to compete in online tournaments on platforms such as CEVO, FACEiT and ESEA.

Due to the amount of players in the Lower Spectrum of Skill Groups here on ZARP, we are implementing a strict recruitment method.
Everyone is aloud to apply, however, you need to meet certain requirements that shall not be listed here.

In order to apply for the team, you must fill out the application form which you will find below, and post that directly as a reply to this thread.

Obviously, Counter-Strike: Global Offensive is a very team based game, so if you think that you won't be able to communicate in a manor that will only benefit the team and not cause friction between teammates, please don't bother applying.

Following on from CS:GO being a team game, one of the simple requirements is that you must be able to speak fluent English, that's not to say that you need to be British, but you do need to know English in order to create fluent communication between Teammates.
