Reason - What is/might be the reason that you got banned? disrespect
Server - What server did you get banned from? SSRP
Time - How long were you banned for? PERMA
Story - What happend? I just said that i will take it to the forums if he warns me for something i never broke since everyone does it unfair much
Proof - Any proof on a video/screenshot that clearly show what happened? Not really
Extra information - Any thing you think may be relevant to your ban. im not even saying sorry this is just making me mad

You cannot debate with the owners how they chose to police their servers. You can apologise and get unbanned but trying to take on an owner about this isn't gonna end well for you bud. Trust me, I know from past experience.
