Correct template looks like this :

Name - What is your ingame name?
STEAM-ID - What is your STEAM-ID?
Admins name - Who banned you/who might it be?
Reason - What is/might be the reason that you got banned?
Server - What server did you get banned from?
Time - How long were you banned for?
Story - What happend?
Proof - Any proof on a video/screenshot that clearly show what heppend?
Extra information - Any thing you think may be relevant to your ban.

Please fill it with correct order like:

Name - What is your ingame name? - MY NAME
STEAM-ID - What is your STEAM-ID? - MY STEAMID ( steamidfinder.com )
Admins name - Who banned you/who might it be? - NICK OF ADMIN WHO BANNED ME
Reason - What is/might be the reason that you got banned? - REASON OF BAN HERE
Server - What server did you get banned from? - ON WITCH SERVER DID I GOT BANNED
Time - How long were you banned for? - FILL CORECT TIME OF BAN ( IF REMEMBER )
Story - What happend? - YOUR BAN STORY
Proof - Any proof on a video/screenshot that clearly show what happend? - SCREENSHOTS, MOVIES SHOULD BE HERE
Extra information - Any thing you think may be relevant to your ban. - EXTRA INFO OR YOUR WORDS HERE
