Admins name - Who banned you/who might it be? His steam name was just Anarchy, as shown in the screenshot
Reason - What is/might be the reason that you got banned? None Whatsoever
Server - What server did you get banned from? ZARP Dark RP
Story - What happend? i was afk for an hour and 30 mins or so checking out a new place im moving into, talking to the landlord and all that, and when i get home i come back to that screenshot

Extra information - What server did you get banned on? How long, was it a permanent ban? What date and time did this happend? Any witness? It was permanant on the DARKRP server,

If you were interrupting RP, and you were AFK, you should've been slain, and then confronted later when you were back.
