Admins Name - Who banned you/who might it be? Console
Reason - What is/might be the reason that you got banned? "Hacking, I though my warning was very clear"
Server - What server did you get banned from? Prophunt
Time - How long were you banned for? Permanent
Story - What happened? I was hiding in a spot on the gas station map in the corner of the map with lots of other props near me, I was standing on top with them surrounding me. I was trying to move further into the corner and I glitched through the wall outside of the map, it made my game lag out massively, I froze and got banned by the console for "hacking".
Proof - Any proof on a video/screenshot that clearly show what happened? none
Extra Information - Any thing you think may be relevant to your ban.
