Admins Name - Who banned you/who might it be? Hiro?
Reason - What is/might be the reason that you got banned? Ban Evading?
Server - What server did you get banned from? SSRP
Time - How long were you banned for? Permanently - it's been around a year
Story - What happened? I was down bad, thought it would amusing to rulebreak. It wasn't funny. I'd like to apologize to anyone who was involved with the situation. I'm sorry.
Proof - Any proof on a video/screenshot that clearly show what happened? N/A
