Reason - What is/might be the reason that you got banned? No idea
Server - What server did you get banned from? Dark rp
Time - How long were you banned for? Perma
Story - What happened? I literally have no idea since it was years ago I think since I have my old name that I have not used in quite a while
Proof - Any proof on a video/screenshot that clearly show what happened? Nope I dont remember even playing just wanna get unbanned since the server looked good when i was running around banned
Extra Information - Any thing you think may be relevant to your ban. Nope

The STEAM-ID (STEAM_1:1:48764934) you linked is not banned. Try reconnecting to either server (usa.zarpgaming.com:27015) or (play.zarpgaming.com:27015) and take a screenshot of the ban reason or make sure you've linked the correct STEAM-ID.
