From what I can see you have only played 30 hours.
You straight up almost have no activity mate only fairly little so if it's because you change names then wait a bit before applying. Lying about time and activity does not help you in any way.

This application looks very rushed.
Please do try and work on it as of right now this just seems straight up bad.

Things you can do better in the application:
Explain all yours rules better and GIVE AN EXAMPLE as stated in it.

Explain all your situations better, you do not even give it a second thought it's just "Rulebreak = warning" you at least talk to players first, explain to them what they did wrong and so on. You don't just warn without investigation unless you saw it happen in person.

You do not at all seem ready to be a mod.
