For when it says when is appropriate to use a ban command. Yes giving them 4 chances is good and all but maybe state some examples and ways you would deal with them
Example -Could you also either bold your questions or use some colors so it makes the application and questions separated.
Over all your game trackers are good but maybe get some time up on server 2 as you are at the minimum requirement (15hrs) for that...

For example - We have a player, he is talking over chat and microphone and for this type of propaganda he is saying 'All black people are slaves' I would act appon this by giving them a warning a gag/mute and consult with further admins for a ban, the ban would last around 5 days.
