This is a great answer but meta gaming is much broader then telling something/someone . In a nut wheel meta gaming is just relaying in character information in a out of character channel and also vice versa.
You can also read the rules if you are unsure on this .

Personally I would verbally warn him )or check his rap sheet to see if he has any meta gaming punishments ) because some people might not be aware of this rule and it is better to let him learn something from that and give him a second chance to improve on his knowledge of the rules.

That is about it there is just one more improvement to your app specifically I can see and that is adding colour and Bolding the questions to make it easier to read .

Personally I have seen you ingame and you seem like a mature and helpful person you do at times seem immature but that is only some times .
