Just seen you on DeathRun so I take it you are applying there. First off you need to follow the hierarchy of Moderator -> Admin -> SuperAdmin -> HeadAdmin/Owner so only fill out the moderator part of the template.

You can find your gametrackers here by putting your in game name in the search bar and then finding your profile for each server:

You can also find your Steam ID here.

Use the edit in the bottom right of your post to make the appropriate changes.

The meeting for DeathRun is Friday at 7 PM GMT (British time) so be sure to make the changes by then.
