I presume you have seen me in game like I have, you. Your activity and gametime is good. I am glad to see you took comments into account and improved your application. You seem mature in-game and in your application and you do not seem to have recent warnings that I remember.

A few things I would suggest doing to your application:

Cutting down on some of the bright colors.
They distinguish the roles well but if they are cut down to something not as bright then it would be easier on the eyes. More so for the green. Eg.
Light Green to Not So GreenMaking your numbered list for acceptable and unaccepted KOS' bold. Eg.
