Anyway allow me to help you in your appeal.
First of all the only real things you need sorted out are the template and your STEAM_ID.

You should be able to edit your post by clicking on the edit post button at the bottom right of your post as Pepper mentioned. Any information you do not have such as proof or extra information you may leave blank if necessary.

Secondly you will need to get your STEAM_ID. To do this please go to your steam profile and right click the page and click copy URL (If using the application) otherwise copy the URL on your browser.
Next you will need to visit steamidfinder.com/ and paste your copied link into it so that it can find your STEAM_ID.
Lastly just copy and paste that into your template and click submit.

Once a member of staff see what your STEAM_ID is I am sure they will check the banlist and see which admin banned you and forward this appeal onto them (hopefully) and they will deal with it and provide their side of the story. Hope this helped I guess.
