I have a couple things that you could improve on:

should make your titles in bold for example:
Should improve this as it would not be FailRP it would be meta gaming
I would also improve this you wouldn't need to ask everybody you would just need to ask for what happened and then if both are different you would need proof (unless you saw him break it).
after looking at how you would handle situations I see you will try get witnesses, witnesses do not count as proof as they could be sticking up for the reported player or reporting player so you need like Video/Screenshot proof so go through your application and over view on what your wrote.
