I have had a look through the logs and matched them up with the video you have supplied, I can confirm you did indeed lose the Warp Trap and need it to be refunded. Please contact a community owner in-game to receive your refund.

This issue should hopefully be fixed by tomorrow, for the time being I can only suggest avoiding using this item briefly, as this seems to be a fairly consistent issue and most of the time it's used, it is just resulting in unnecessary loss of the item.
