There is no possible ways for items to "randomly" disappear. Is there any possible way you could of lost the item through double equipping, Logging off with the item, Switching jobs with the item equipped for died and didn't notice having it equipped. Apart from that I will still forward this to a cm to check the logs on when you possible lost it.
