I highly doubt there was a "bug", all I can think of is that you pushed someone with your crowbar and then they somehow died in the lava, that would show as you killing them which is pretty stupid at times.

Anyway, the ban is not solely off of this one punishment as mentioned, and it's pretty clear you have an awful rapsheet as I included it in my verdict.
