Story/Situation -
Now, I got the warning without any dialog, or what ever.
The nade wasn't random it was throw to kill a T that was in the Nether,
now I get that it might look like a Random Nade, but here the following fact that happend:

The Traitor (monu or something, was seen in the traitor room, I proceeded to follow him and SAW him go into the nether, James and I (both detectives) said he's in the nether we both procceded towards the portal and I thew a Incend grenade to make sure when he comes out he'd die. the granade did NOT go through the portal so there for I called out "Watch out theres a nade in front of the portal" vocally and the T that was in the nether jumped out the nether and died by my incend grenade and followed by reporting me for RDM.

Extra Information - If I actually would of threw the nade randomly i'd own up to it.
