For your explain RDM you have not actually explained what it is you have only given an example and even as an example its confusing and a little wrong. You make it sound like because both players were innocent then it is always RDM, which is not the case, an innocent may kill an innocent and it not be RDM, by using your example of the diamond block if someone is taking it away from the furnace you can kill them for it and not be warned if they were innocent.
For your first unacceptable KOS reason its again very confusing. If you mean you can't KOS someone mid fight then yes and no, it depends on the situation. If you mean a player is attacked, kills the attacker and the attacker was an innocent, then even if it was self defence the killer can be KOSed.
