Your application is good and it is clear that you have put effort into it however it could be improved. For example, your 2nd acceptable KOS reason could be improved by giving an example scenario, something like Player A watches Player B kill someone and enter the t room, Player A manages to see their name and heads to the nether portal, killing Player B when they come out.
