On top of that 6 of those punishments are for prop killing, which is an intentional rule break, its not like RDM where accidents can happen, you are purposefully pushing someone with a cart, you are purposefully breaking rules. As a member of staff you will be the one enforcing the rules so if you refuse to follow them and intentionally break them we have a big problem.

As for your application my initial thoughts are that you did not put a lot of time into it. The template states that you should give 3 examples of an acceptable kos and 2 examples of an unacceptable kos, you have only put 1 for each. Also i agree with Nigerian that they are very unclear and could use some extra work.
