In order to apply for moderator on any server (death run in this case) You need to fill out the following template, press the fancy "edit" button then paste the template in and fill it out with your information. I'll move your app to the "server applications" section in a moment as well as update the title next time you want to post your apps in that section and put the prefix to [SERVER NAME, RANK] Yourname's application.
