I have looked into the report and came to the following conclusion

Conclusion

The Admin in question was using
 a innocent rp feature and misused it to block of a entrance. 
6.3 You may not block any entrances to the vault. All entrances must be accessible.

I have already spoken to the admin and he understand to not do this again
