Reason - What is/might be the reason that you got banned? DISRESPECT
Server - What server did you get banned from? WARNED FROM:l SSRP 1
Time - How long were you banned for? N/A
Story - What happend? Cyber had the wrong steamid and i want the warning removed from my f6 menu

Aslong as the warning was removed it isn't included as a recent warning, include this in your application so if it doesn't get removed it will be counted as a invalid punishment which wont be counted as something that can get you denied. Kylie did that to me but i still got denied when i applyed for mod some time ago.
